About The Position

The Senate is a permanent supportive housing residence for formerly homeless adults, many with mental illness, substance abuse/dependence and co-morbid medical diagnoses. On-site services include medication monitoring, case management, money management, entitlement assistance, medical and psychiatric care, substance abuse counseling, work readiness training, 24 hour/7 day per week front desk security, and group activities. The Senate is seeking a dynamic individual to provide case management services to formerly homeless adults diagnosed with a severe persistent mental illness in order for tenants to maintain permanent housing as well as to utilize client centered approach to assist tenants with working towards their goals.

Requirements

  • Bachelor’s Degree in social work or a related field required
  • Understanding of housing first model
  • Understanding of severe and persistent mental illness
  • Understanding of harm reduction and trauma informed care
  • Ability to multitask
  • Strong crisis management skills
  • Strong interpersonal skills
  • Strong written and verbal skills
  • Team player
  • Proficient in Microsoft Outlook and Excel
  • Able to use or learn computer databases such as AWARDS

Nice To Haves

  • Experience working with individuals diagnosed with severe and persistent mental illness, substance abuse, and/or co-occurring disorders preferred
  • Spanish speaking preferred

Responsibilities

  • Provide case management to a group of assigned clients; identify short and long-term treatment goals and help clients achieve them, including linkages to community programs such as medical, psychiatric and drug treatment services
  • Coordinate and follow up with the onsite medical and psychiatric treatment providers
  • Provide onsite medication monitoring to tenants
  • Accompany clients on visits to doctors, entitlement offices or other community services
  • Maintain case records with documentation of services provided and outcomes, according to specific program procedures
  • Assist clients in receiving entitlements and in understanding and complying with building rules and government subsidies to maintain residency
  • Collaborate with staff in other GRCC programs to coordinate client service
  • Encourage participation among resistant clients; conduct floor meetings to introduce and encourage program participation
  • Attend staff meetings, case conferences, training programs and supervision meetings
  • Participate in planning and leading group activities
  • Collaborate to maintain an active resource directory
  • Track and maintain all data outcome measures on tenants physical, mental and behavioral health with a results orientated framework
  • Other duties as required include conducting unit inspections, and reviewing changes in government policies as well as changes in pharmaceutical and insurance companies
