With a focus on collaboration, the CFSI team maintains manageable caseloads and works closely with schools, courts, and community partners to provide strength-based guidance and informed decision-making. The Case Manager plays a pivotal role in empowering youth and families by assessing their needs and crafting personalized success plans alongside the CFSI Director and Youth Coaches. This position involves connecting families to essential community resources like mental health services, substance abuse treatment, education, and employment opportunities. The Case Manager ensures seamless coordination through youth-centered team meetings, timely documentation, and collaboration with DSS, Probation, and the Court. They also represent the agency in Family Court, County Advisory Boards, and Coalitions, championing the voices of the youth they serve. With a deep sensitivity to trauma and unique service needs, this role is at the heart of driving impactful change and fostering brighter futures.
