About The Position

The Ryan White Program Medical Case Manager is responsible for the overall coordination of medical and ancillary services for HIV/AIDS patients of the Ryan White Program while serving as liaisons to other medical and social service providers and community agencies. The Medical Case Manager is responsible for the overall coordination of care for the patient including comprehensive medical case management addressing all their medical and human service needs. The medical case manager participates in the patient`s clinical care plan, case conferences with providers to provide relevant updates that may impact the progress of the care plan. The Medical Case Manager works closely with the patient to guide, assist, and ensure needed services are properly accessed/received in a timely fashion. At minimum, the Medical Case Manager`s responsibilities include the following: Assessment, Care Planning, Service Coordination, Advocacy, Progress Monitoring, Chart [EMR] Documentation, Crisis Intervention, and Community Engagement and Collaboration.

Requirements

• Bachelor`s Degree in Social Work, Public Health or health related field, Associate Degree in health related field, or confirmed five years experience in direct HIV client/patient care (outreach, engagement and retention in HIV care).
• Monday through Friday - 8:00 AM - 4:30 PM No weekends.
• Influenza Vaccinations are a requirement for employment. If you are not currently vaccinated you will be required to receive the vaccination prior to hire date, during the influenza season, if you are offered employment, unless you request and receive an approved medical exemption.
