About The Position

The Case Manager provides comprehensive case management services across ILP/SLP and Residential programs, ensuring the health, safety, rights, and quality of life of individuals served. This role coordinates clinical, medical, residential, and community-based supports while emphasizing person-centered planning, consumer advocacy, regulatory compliance, and collaboration with families, providers, and interdisciplinary teams. Working in partnership with the Assistant Director and Director of Residential Services, the Case Manager provides broad oversight of assigned residential, shared and independent living settings. The role serves as a professional role model and effective team leader, using collaboration and clear communication to support consistent, high-quality program operations. The Case Manager maintains the clinical integrity of services and ensures that individuals' developmental, physical, emotional, and social needs are met within a Positive Behavior Support (PBS) framework. The position supports residential leadership with program management responsibilities and plays an active role in continuous quality improvement, best practice implementation, and the ongoing development and enhancement of residential and supported living programs.

Requirements

  • Bachelor's degree in human services or a related field required.
  • Minimum of two (2) years' experience working with adults with developmental disabilities, elderly adults, or similar populations.
  • Strong written and verbal communication skills.
  • Ability to work independently with sound judgment, organization, and time management.
  • Proficiency with basic computer applications, including word processing and spreadsheets.
  • Valid driver's license, acceptable driving record, and reliable transportation.
  • Ability to obtain and maintain required certifications (CPR, First Aid, Medication Administration, Human Rights, Fire Safety, and related trainings).

Nice To Haves

  • Massachusetts Social Work license preferred where applicable.
  • Experience in residential, supported living, or community-based service models preferred.

Responsibilities

  • Ensure individuals are treated with dignity and respect in accordance with Minute Man Arc Human Rights Policy and DDS regulations.
  • Safeguard individual rights, privacy, confidentiality, and freedom from abuse or neglect.
  • Respect and support cultural, ethnic, and religious preferences.
  • Serve as a mandated reporter and complete required incident reporting in accordance with DPPC and agency policies.
  • Support individuals in self-advocacy and meaningful decision-making related to daily life, services, and long-term goals.
  • Participate in and support Individual Service Plan (ISP) development, implementation, and review.
  • Ensure ISP timelines, documentation, and follow-up requirements are met.
  • Maintain accurate, timely, and confidential records in compliance with agency, DDS, AFC/SL, and other regulatory requirements.
  • Complete assessments, progress notes, and required reports within established timeframes.
  • Communicate significant health, behavioral, psychiatric, or environmental concerns promptly to supervisors and relevant stakeholders.
  • Provide crisis stabilization, clinical risk management, and on-call or emergency response as assigned.
  • Coordinate emergency services and hospitalizations when necessary.
  • Maintain required certifications and complete all mandatory trainings.
  • Participate in supervision, team meetings, and required clinical or administrative meetings.
  • Use agency resources responsibly and support cost-effective practices.
  • Conduct initial and ongoing assessments of ILP/SLP living environments to ensure suitability, safety, and regulatory compliance.
  • Complete psychosocial assessments and social histories as required.
  • Conduct required on-site visits and maintain corresponding progress notes.
  • Monitor provider settings, including fire and safety standards, and document continued suitability at least annually or as indicated.
  • Review provider documentation and logs to ensure quality of care and compliance.
  • Support community integration, independence, and skill development consistent with ILP/SLP models.
  • Participate in discharge planning and transitions as appropriate.
  • Coordinate and oversee healthcare needs, including scheduling and tracking medical, dental, psychiatric, and therapeutic appointments.
  • Support nutrition planning and encourage balanced meals with individual participation.
  • Communicate regularly with families and guardians regarding individual progress and concerns.
  • Provide oversight of assigned residential programs in collaboration with Residential Leadership.
  • Monitor daily supports through review of T-Log, GERs, data collection, and outcome measures.
  • Support positive behavior supports and ensure clinical integrity of the residential program.
  • Ensure accurate medication management documentation, including physician orders, transcription, refills, and controlled substance counts.
  • Facilitate house meetings and support meaningful community inclusion, activities, and social engagement.
  • Collaborate closely with Residential Leadership, nursing, behavioral health, and external providers.
  • May assist with delegation of tasks to direct support staff but does not carry primary supervisory responsibility unless assigned.
  • Support hiring processes and training recommendations as requested.
