About The Position

The Case Manager provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulations, as established by the State Board of Registered Nursing, the patient and the policies of Harbor Hospice. Care Manager facilitates coordination and communication between hospital, patient and families and IPU to expedite admission and assess acuity. Case Manager creates & oversees Plans of Care for each patient admitted to the inpatient unit.

Requirements

  • Registered Nurse with degree from an accredited nursing school and holds a current license in the State of Michigan.
  • Possesses and maintains a current CPR certification.
  • Maintains a valid Michigan driver’s license and has the availability of a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Responsibilities

  • Knowledgeable of hospice concept and philosophy.
  • Assess patient appropriateness for hospice services. Assess patient/family/care giver and develop plan of care including safety and teaching. Completes LCD at recertification. Prepares for and collaborates in an effective discharge plan.
  • Reports in weekly interdisciplinary team meetings. Participates in team decisions concerning patients including designating patient acuity. Assess for GIP status prior to admission and at least daily. Communicates individually or within a group to keep team advised of patient status.
  • Participates in information visits at the hospital for potential admissions to the inpatient unit. Coordinates admissions with intake nurses/home care case managers for community admissions and transfers. Obtains/provides report to IPU team prior to admission as needed.
  • Assists with admitting new patients and is responsible for ensuring thoroughness of the documentation
  • Provides/promotes excellent communication with families, patients and visitors. Follows up with concerns and brings unresolved issues to clinical manager.
  • Performs and is knowledgeable of venipuncture and port lab draws; IV starts, and CADD pumps as necessary.
  • Knowledge related to use of IV and external feeding pumps; operations and maintenance.
  • Completes appropriate charting including daily GIP charting and all RHC for assigned patients. Maintains visit string updates.
  • Follows up with appropriate staff to ensure missing/inaccurate information is completed.
  • Manages medication including ordering meds, ensures proper orders in EMR.
  • Demonstrate proper hand washing, asepsis, and standard precautions.
  • Demonstrate and teach patient/family/care giver proper usage of DME and supplies.
  • Provide CENA supervision in accordance to Medicare regulations.
  • Orient staff and volunteers as assigned.
  • Provide in-service and educational programs to staff and other health professionals as needed.
  • Rounds with physician: coordinates appropriate information related to patient’s plan of care.
  • Organizes/manages regulatory requirements including: F2F visits due, SFV, HUV and weekend admit notification.
  • Maintain credentials at area hospitals.
  • Serves as Case Manager for GIP patients at the hospital.
