About The Position

The Case Manager position at Gateway CSB involves providing ongoing supportive and case management functions to consumers with long-term mental illness. Requirements

  • Bachelor's degree from an accredited program.
  • Minimum of 2 years related experience in case management or mental health services.
  • Valid GA Driver's license.
  • Working knowledge of serious mental illness and behavior modification techniques.
  • Knowledge of consumers' rights and agency policies.
  • Ability to communicate effectively with diverse populations and maintain confidentiality.

  • Experience with crisis intervention techniques.
  • Familiarity with community resources and service delivery systems.

Responsibilities

  • Provide ongoing supportive and case management functions to consumers with long-term mental illness.
  • Participate in interdisciplinary staff meetings to coordinate care for consumers.
  • Document services accurately and maintain compliance with agency standards.
  • Ensure all clinical documentation is completed according to agency requirements.
  • Maintain a minimum chart audit score of 70% for all consumers on the case-load.
  • Collaborate with behavioral health providers and community resources to transition consumers through care systems.
  • Meet minimum direct time requirements for billed hours per year and enroll individuals into case management services.
  • Conduct a minimum of 2 face-to-face visits monthly with clients.
