About The Position

Performs highly advanced case management work. Work involves coordinating and evaluating case management and program activities; developing program goals, objectives, and procedures; evaluating outcome measures for the program; and supervising the work of others. Works under minimal supervision with extensive latitude for the use of initiative and independent judgment.

Requirements

  • Graduation from an accredited senior high school or equivalent or GED.
  • Five years full-time, wage-earning case processing, case management, social services, criminal justice, program administration, program evaluation, peer support, human services, technical program support, or operational support experience. Fifteen semester hours from a college or university accredited by an organization recognized by the Council for Higher Education accreditation (CHEA) or by the United States Department of Education (USDE) may be substituted for each six months of experience.
  • Experience in the supervision of employees preferred.
  • Knowledge of case management principles and practices.
  • Knowledge of case management objectives, standards, and methods.
  • Knowledge of community resources.
  • Knowledge of agency and departmental organizational structure, policies, procedures, rules, and regulations preferred.
  • Skill to communicate ideas and instructions clearly and concisely.
  • Skill to coordinate with other staff, departments, officials, agencies, organizations, and the public.
  • Skill to interpret and apply rules, regulations, policies, and procedures.
  • Skill in problem-solving techniques.
  • Skill to prepare and maintain accurate records, files, and reports.
  • Skill to review technical data and prepare technical reports.
  • Skill to plan work in order to meet established guidelines.
  • Skill in monitoring service and treatment plans.
  • Skill to utilize available community resources.
  • Skill to assess offender needs and coordinate offender services.
  • Skill to supervise the work of others.

Responsibilities

  • Coordinates case management, program activities, and documentation; monitors and evaluates service delivery systems and inmates’ progress; and participates in the quality assurance and utilization review processes for case management to ensure quality services.
  • Documents, reviews, and edits case records, reports, and evaluations; and coordinates transition planning to ensure continuity of care and management of risk factors.
  • Coordinates the development and implementation of clients’ service and treatment plans; makes recommendations; and monitors case management for clients enrolled in the program.
  • Provides ongoing case management and serves as a liaison with inmates, inmate families, and service providers; implements follow-up and aftercare plans for inmates; and disseminates information on community resources for inmate placement in follow-up treatment.
  • Supervises the work of others; participates in training events; and provides technical assistance to others.
  • Performs a variety of marginal duties not listed, to be determined and assigned as needed.
