About The Position

The Case Manager for the Independent and Transitional Living Program supports at-risk youth and young adults ages 14–24 in achieving safe, stable, and independent futures. Working under the direction of the Program Coordinator, the Case Manager provides individualized support through planning, resource connection, and skill development. This role ensures that youth receive guidance, structure, and access to essential services while reinforcing personal accountability and long-term goal achievement. Success in this position requires the ability to identify and close knowledge gaps (e.g., life skills, systems navigation), reinforce the importance of personal growth and responsibility, and take timely, consistent action in service delivery and documentation.

Requirements

  • High school diploma or GED required; some college coursework in human services or related fields preferred.
  • One to two years of experience working with at-risk youth, social services, or case management highly preferred.
  • Strong organizational, communication, and interpersonal skills.
  • Must be able to pass a background check and UA
  • Demonstrated reliability, flexibility, and ability to maintain professional boundaries.
  • Valid driver’s license with reliable transportation for client-related travel.

Responsibilities

  • Assist youth in developing and executing individualized case plans focused on education, employment, housing, and life skills.
  • Conduct regular client check-ins, both in person and through approved communication channels.
  • Provide transportation and referrals to community resources, including employment, housing, healthcare, and education.
  • Deliver life skills training and reinforce concepts related to budgeting, nutrition, hygiene, and self-care.
  • Support clients in understanding and navigating systems such as workforce services, public assistance, and housing.
  • Complete and maintain accurate, timely documentation in HMIS, BestNotes, IL Tracking, TLP Tracking, and Client Contact Logs.
  • Submit monthly client reports and ensure all files are current and audit-ready.
  • Record client entry and exit information in all required systems.
  • Track attendance and participation in required client group meetings and workshops.
  • Ensure compliance with grant expectations and internal documentation standards.
  • Collaborate with schools, workforce agencies, and other providers to build support networks for clients.
  • Conduct outreach weekly to the Council of Community Services and monthly to other key partners.
  • Distribute program materials (flyers, brochures) and participate in street outreach to identify eligible youth.
  • Promote the program’s value and encourage ongoing participation among eligible and enrolled clients.
  • Maintain ongoing communication with Housing Managers and coordinate client needs.
  • Participate in a rotating on-call schedule as assigned.
  • Accept and complete other duties as assigned by the Program Coordinator.
  • Demonstrate flexibility and adaptability as program needs evolve.
