About The Position

Requirements

• Bachelor's degree in Social Work
• Three years of experience in a social work setting
• Valid Driver's License
• Strong communication skills

Responsibilities

• Provide comprehensive case management services.
• Set up and maintain case files in accordance with standards.
• Develop Treatment Plans.
• Conduct Treatment Plan reviews.
• Conduct admissions and discharges.
• Serve as liaison with CPS social workers, family members, and others.
• Conduct and document monthly wellness checks.
• Coordinate arrangements for all medical care.
• Assist with supervision of residents as needed.
• Assist with rotating on-call duties.
• Provide after care services on a scheduled basis and as needed.
• Plan and implement weekly skills groups.
• Maintain a valid driver's license and a clean driving record.
• Maintain personal automobile liability insurance ($50K minimum).
• Other duties as assigned and mutually agreed upon.
