About The Position

The Supportive Housing Case Manager is responsible for providing guidance and extensive support to families facing homelessness, substance use, mental health issues, and domestic violence. Utilizing a 'housing first' model, the Case Manager collaborates with Housing Specialists to secure housing for clients and assists them in building skills and income to maintain their housing. The role involves conducting assessments, facilitating family meetings, and connecting clients with necessary services, while also acting as a liaison with Child Welfare Services.

Requirements

  • Bachelor's or AA Degree in a related field or 3 years of case management experience.
  • Experience working with Child Welfare/Child Protective Services.
  • Experience with homeless or at-risk families, dual diagnosis, substance abuse, and domestic violence.
  • Skilled in crisis intervention and conflict resolution.
  • Ability to communicate effectively both verbally and in writing.

Nice To Haves

  • Master of Social Work (MSW) degree preferred.
  • Knowledge of San Francisco low-income housing.

Responsibilities

  • Provide intensive case management and support to families receiving housing subsidies.
  • Conduct psychosocial assessments to develop treatment plans for families.
  • Offer crisis intervention and support through home visits and treatment visits.
  • Educate families about substance abuse, domestic violence, mental health, and parenting.
  • Assist families in achieving goals such as stable housing, sustained recovery, and employment.
  • Utilize motivational interviewing and counseling to improve family functioning and stability.
  • Monitor client subsidies and ensure compliance with program policies.
  • Develop action plans with clients and connect them with community services.
  • Input required data into the HPP Salesforce database and maintain accurate documentation.
  • Attend case conferences, team meetings, and individual supervision meetings.
