About The Position

Cortelyou is a 46 unit supportive housing program located in Flatbush Brooklyn. The Program specially services young adults 18-25 years old with Serious and Persistent Mental Illness and have a chronic history of being unhoused. Most of the young adults living at Cortelyou are members of the LGBTQ+ community. The Program offers a safe environment and supportive services to help our young adults to move towards long term stability. This Program is under the NY/NY 15 population C agreement and funding is being provided through the Department of Health and Mental Hygiene.

Requirements

  • BSW or Bachelor's in human services related field, with a minimum of two years of experience with homeless, at-risk, or system involved LGBTQIIA youth, experience serving youth living in supported housing.
  • Strong written and verbal communication skills.
  • Ability to build relationships with in-house colleagues and external partners.
  • Strong organizational, time-management, and technical skills.
  • Propensity and willingness to learn, persevere, and think outside the box to help young people make progress toward their goals.
  • Undergo pre-employment screenings such as the Criminal Background Check required by OMH.

Responsibilities

  • Cultivating a safe and affirming living environment for formerly homeless, young adults, many living with a significant mental illness, and identifying as part of the LGBTQIA+ community.
  • Providing consistent, high quality program service delivery to all clients populations in accordance with funder and regulatory requirements; best practices; BCS policies and Core Values; and Code of Conduct for Custodians of People with Special Needs.
  • Navigating all client services including referrals for specialty care, coordinating appointments, and aiding residents with achieving goals identified in their Individualized Treatment Plan.
  • Completing all required assessments and service plans in a timely manner.
  • Completing progress notes within 24 hours of individual sessions.
  • Meeting with all clients assigned to caseload a minimum of once per week and/or as instructed by Program Director.
  • Ensuring all clinical and administrative documents are completed accurately, timely and in accordance with program policies/procedures and DOHMH regulatory standards.
  • Providing crisis intervention services and completing all incident reports in accordance with DOHMH standards.
  • Working with residents in their homes, the local community, and program offices to elevate their overall daily living skills.
  • Escorting clients to appointments as instructed by Program Director.
  • Collaborating with property management to address all client apartment needs and repairs.
  • Conducting apartment inspections as instructed by Program Director.
  • Collaborating with MHANY management to assist clients with navigating all processes required to recertify apartment units.
  • Aiding with coordinating resident move-ins and move-outs.
  • Participating in recreational events in the community to increase consumer socialization skills.
  • Attending all meetings, trainings, seminars, workshops, and case conferences as assigned and required.
  • Completing all other tasks as assigned by Program Director.
