About The Position

The Case Manager for Support Services at Richcroft is responsible for overseeing personal supports, respite, shared living, and supported living services within the Support Services Department. This full-time position involves managing a caseload of individuals with developmental disabilities, ensuring quality service delivery, and collaborating with external organizations to develop and monitor Person-Centered Plans (PCP).

Requirements

  • An Associate's degree in Human Services, Social Work, Psychology, or Education is preferred.
  • At least two years of experience in the field of developmental disabilities.
  • Current driver's license (not provisional) with less than 3 points on driving record.
  • Access to personal use of a vehicle for agency business.

Responsibilities

  • Supervise 25-35 individual cases and direct care staff.
  • Provide case management services as per policies, procedures, and regulations.
  • Monitor the quality of services via visits and communication with each person/family receiving support services.
  • Collaborate with external organizations on the development of the annual Person-Centered Plan for each person receiving supports (PCP).
  • Monitor progress of the outcomes from the PCP monthly and complete a progress report every 90 days for all individuals on caseload.
  • Complete progress notes for all individuals on the caseload as outlined in the Individual Plan and submit to the director.
  • Maintain all program files following established guidelines.
  • Provide timely and effective responses to calls.
  • Provide support, training, and day-to-day guidance and direction to staff and individuals of Richcroft, Inc.
  • Keep the Director informed of any unusual events involving staff, individuals, or the community in a timely manner.
  • Communicate with external organizations as needed.
  • Maintain all required licenses and certifications.
  • Conduct bi-monthly formal supervision with direct support staff and maintain supervision documentation.
  • Conduct bi-weekly supervision with Director and review payroll.
  • Weekly monitoring of LTSS for EVV accuracy and make corrections as needed.
  • Review and process staff reimbursement requests.
  • Participate in staff interviews, hiring, and onboarding process.
  • Monitor and update staffing needs.
  • Maintain Outlook Calendar and working schedule to complete duties as required.
  • Fill in for staff who are out as needed.
