About The Position

We are looking for a Case Manager who is willing to embrace McKinley's mission and HUMAN values. The role of the Case Manager is to work in concert with the treatment team and particularly the STRTP Clinical Social Worker to liaison with the residents, families, and county workers with regards to the progress and treatment issues of the individual clients.

Requirements

  • Bachelor's Degree or Associate's degree plus four years of experience
  • DMH experience preferred
  • Residential experience preferred
  • Proficient in Microsoft Office Suite (MS Word, Excel, PowerPoint, Outlook, and other office-related software)
  • Possess superior oral and written communication skills as well as interpersonal skills
  • Detail-oriented and organized
  • Able to work independently and organize multiple projects with minimum supervision
  • Able to work as a team member
  • Able to maintain the highest level of confidentiality and professional boundaries with clients and staff
  • Able to represent the agency professionally and appropriately with all constituencies
  • Must pass pre-employment physical exam, TB, and drug screening
  • Ability to commute to various sites on and off campus
  • Ability to lift 10 pounds for filing
  • Must complete crisis prevention/intervention training (Pro ACT) within the first 90 days of employment

Responsibilities

  • Maintain a caseload of clients and follow them from admission to discharge
  • Responsible for completing appropriate documentation
  • Assists with the implementation of clinical interventions and participates in an interdisciplinary treatment team for the clients on their caseload
  • Facilitates psycho-educational groups for the clients at the facility when indicated
  • Responsible for contributing to the implementation of client behavior plans
  • Engaging in crisis intervention when required and ensuring consistently high quality of client care to an at-risk youth population
