About The Position

The Case Manager (CM) will support a caseload of vulnerable individuals experiencing houselessness by helping them overcome barriers to sustainable housing. This role involves assisting participants with goal identification, completing necessary documentation, connecting them to resources for wellness, and facilitating community integration. The CM will work within a program team that employs strengths-based, housing-first, and harm-reductive models to enhance participants' health and well-being.

Requirements

  • Experience in program management or case management.
  • Knowledge of procedures and techniques for effective case management.
  • Ability to work with individuals with serious and persistent mental illness and/or substance use disorder.
  • Experience with trauma-informed care and de-escalation techniques.
  • Strong communication skills, both verbal and written.
  • Ability to work independently and manage multiple tasks in a fast-paced environment.
  • Detail-oriented with strong organizational skills.
  • Commitment to DEIA principles and practices.

Nice To Haves

  • Experience with the Homeless Management Information System (HMIS).
  • Familiarity with community resources for veterans and non-veterans.

Responsibilities

  • Conduct weekly case management meetings with all participants on your caseload.
  • Engage with participants to assist them in identifying individual goals for stable housing.
  • Advocate for participants' needs consistently.
  • Assist participants in applying for eligible benefits such as medical insurance and food stamps.
  • Document all engagement with participants and their health and well-being accurately.
  • Maintain up-to-date physical and electronic records using HMIS and CaseWorthy.
  • Develop knowledge of community resources and maintain effective relationships to meet participant needs.
  • Deliver hygiene kits, food boxes, and essential supplies to participants.
  • Assist with shared site operation duties like meal distribution and cleaning tasks.
  • Maintain a safe and sanitary environment for all participants and staff.
  • Communicate clearly and respectfully with staff and community partners.
  • Attend assigned trainings and staff meetings.
  • Participate in compliance and quality assurance activities.
