The Case Manager at WestCare is responsible for providing comprehensive case management services to program participants, particularly those struggling with substance abuse and those in recovery. This role involves collaborating with both WestCare staff and external organizations to ensure that participants receive the necessary assessment, counseling, re-integration, and rehabilitation services. The Case Manager will assess the needs, conditions, and desires of assigned participants, and will play a crucial role in educating and referring them to community agencies that can assist with their needs for food, shelter, clothing, employment, and transportation. In addition to direct service provision, the Case Manager acts as an advocate for participants, working to improve their access to vital services. This includes participating in clinical staffing decisions to evaluate the strengths and resources of participants, as well as their need for additional support. The role requires timely documentation of services provided, input into treatment needs, and participation in case conferences. The Case Manager will also prepare discharge summaries as needed and assist other staff members to promote a cooperative and professional image of WestCare in the community. The Case Manager is expected to embody the mission, vision, guiding principles, and clinical goals of the WestCare Foundation, and to perform any other duties as assigned. This position is full-time and operates during the day shift, providing a structured work environment focused on supporting individuals in need of assistance.
