Responsibilities

• Provide resources and support to individuals with intellectual and developmental disabilities and their families.
• Assist individuals with applying for financial assistance and accessing various services such as healthcare, vocational, and educational resources.
• Advocate for services that support individual success.
• Maintain all case records and prepare person-centered plans.
• Coordinate meetings with individuals, families, and interdisciplinary team members.
• Establish and maintain positive relationships with individuals, families, and state officials.
• Track, monitor, and enter specified data points related to case management.
• Report any suspected abuse, neglect, or exploitation immediately.
