About The Position

The Community Outreach Case Manager works alongside Grants Pass Police to engage proactively with community members who may have behavioral health needs. The case manager works to connect individuals with the resources they need including mental health and substance abuse treatment services available through Options. The Community Outreach Case Manager is knowledgeable of and works with social agencies and service providers in the community for the benefit of the people they serve. This position works directly in the community (downtown area, parks, community partner locations etc.) both with local law enforcement as well as independently to connect and engage those in need of behavioral health and other services.

Requirements

  • Bachelor’s (BA/BS) degree in a human services field OR three years of experience providing services in a similar setting with individuals who have emotional and/or mental and/or developmental disabilities, OR a combination of education and experience that provides the desired skills, knowledge, and abilities required to perform the job.
  • Must be a QMHA on the Mental Health & Addictions Certification Board of Oregon (MHACBO) registry.

  • Candidates must be able to use e-mail, compose documents, save and locate documents electronically, and learn to use our Electronic Health Record (EHR) system.
  • Must pass both a state-required background check and a modified law enforcement background check, and DMV checks; possess a valid OR driver's license and a satisfactory driving record to transport clients.
  • Candidate must be able to work independently and flexibly, under general supervision.

Nice To Haves

  • Bilingual skills a plus; Options pays a premium for employees who can conduct business in Spanish and provide certification of a language proficiency test approved by the Oregon Health Authority that meets a proficiency level of “High Intermediate”.
  • Proficiency in Word and other MS Office applications preferred.
