About The Position

The Support Coordinator provides the activities necessary to help individuals with intellectual and developmental disabilities (IDD), physical disabilities, or aging‑related needs to access the services and supports that allow them to live independently. This role is ideal for someone who is organized, detail‑oriented, and passionate about helping others navigate complex systems with dignity and choice. You will work as part of a collaborative care team that values compassion, problem‑solving, and high‑quality service delivery. Care Lync is a social services agency dedicated to providing support coordination and service navigation for individuals enrolled in Medicaid waiver programs. Requirements

  • BS/BA in Human Services, Social Work, Psychology, Education, or related field, or equivalent experience in people‑focused roles
  • 1+ years of experience supporting, coordinating, or assisting individuals with disabilities and/or aging adults in accessing services, resources, or community support.
  • Experience with Microsoft Office and comfort learning new databases
  • Ability to maintain required state clearances and complete all training
  • Valid driver's license, vehicle insurance, and current registration (if travel is required)
  • Strong communication, organization, and follow‑through skills
  • Ability to work independently while contributing to a supportive team environment

Responsibilities

  • Assist participants in identifying their needs and goals related to housing, health, employment, and community engagement.
  • Help develop and update support plans based on changes in a participant's medical, social, or personal circumstances.
  • Coordinate communication between participants, families, providers, and community agencies to ensure smooth access to services.
  • Support participants with scheduling, reminders, and follow‑up tasks related to medical, behavioral health, and community services.
  • Research and connect participants to resources such as transportation, benefits, home‑ and community‑based supports, and social programs.
  • Maintain accurate documentation in state‑maintained databases and internal systems.
  • Build strong, professional relationships with participants and their support networks.
  • Participate in required training, certifications, and ongoing professional development.
  • Monitor participant satisfaction and wellbeing, elevating concerns to supervisors as needed.
