  • Provide intensive support services to individuals recently discharged from the hospital or those at risk of hospitalization.
  • Link clients to HNNCSB and community resources, navigating both physical and mental health needs.
  • Teach essential coping skills to clients.
  • Ensure continuity of services for mental health, developmental disabilities, and substance use disorder populations.
  • Assist clients with their basic physical, financial, social, and personal needs.
  • Conduct outreach, psychological assessments, treatment planning, and advocacy.
  • Provide crisis intervention and medication supervision.
  • Meet with clients, families, and care providers to assess needs.
  • Prepare mental health status summaries and respond to inquiries about available services.
  • Participate in discharge planning and monitor client progress.
  • Bachelor's degree in Human Services such as Social Work, Psychology, Sociology, or Counseling.
  • One year of relevant case management experience.
  • Strong communication and interpersonal skills.
  • Valid Virginia Driver's License.
  • Proficiency with case management software and tools.
  • Excellent organizational skills.
  • Solid understanding of mental health, developmental disabilities, and substance use disorders.
