About The Position
The Case Manager for the Roads to Recovery (R2R) program within the Department of Public Health and Environment for the City and County of Denver plays a crucial role in supporting individuals impacted by the criminal justice system who are experiencing homelessness, mental health issues, and substance use disorders. This position is part of a broader initiative aimed at diverting individuals from the criminal justice system and facilitating their reintegration into society through coordinated intervention and treatment. The Case Manager will manage a caseload of clients, providing comprehensive support and resources to help them navigate their challenges and achieve stability. In this role, the Case Manager will work closely with a team of R2R case managers, supervisors, and community partners to create a coordinated response to the needs of clients. Responsibilities include conducting outreach and in-reach activities, performing client check-ins, collecting and tracking data, and managing client placements and referrals. The Case Manager will also be responsible for maintaining positive relationships with clients and community partners, ensuring effective communication and collaboration. The position requires a strong commitment to fostering an inclusive culture and implementing equitable public health strategies. The Case Manager will actively participate in community outreach efforts, support clients through a wrap-around care management model, and act as a liaison between program management and community partners. This role is essential in helping clients access treatment placements and other necessary resources, ultimately contributing to their recovery and reintegration into the community.
