As a Field Case Manager RN with UnitedHealth Group, you'll support a diverse member population with education, advocacy and connections to the resources they need to feel better and get well. This is a field-based role.

Requirements

  • Current, unrestricted RN license in the state of Connecticut
  • Certified in Basic Life Support
  • 1+ years of clinical experience
  • Reside in or near New Haven, Connecticut
  • Access to reliable transportation

Nice To Haves

  • BSN
  • Certified Case Manager (CCM)
  • Case management experience
  • Experience in discharge planning
  • Experience in utilization review, concurrent review or risk management
  • Experience in a telephonic role
  • Experience in home care/home visits
  • Experience working with MS Word, Excel and Outlook
  • Background in managed care
  • Bilingual English/Spanish

Responsibilities

  • Visit members in their homes to assess their current health status
  • Identify gaps or barriers in treatment plans
  • Provide patient education to assist with self-management
  • Make referrals to outside sources
  • Provide a complete continuum of quality care through close communication with members via in-person in home or on-phone interaction
  • Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels
