About The Position

The Case Manager RN utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population in order to facilitate patients through the continuum of care. Case Manager RNs are responsible for the following: Advocates for the patient and family throughout the entire episode of care. Monitors patient’s progression intervening as needed to ensure efficient patient progress. Coordinates all aspects of discharge planning and establishes a safe discharge plan.

Requirements

  • M-F 8:430 1 weekend day shift every 4 weeks 1 major holiday
  • Willingness to occasionally float to all 3 campuses
  • Required: BSN
  • 2 years of medical surgical nursing experience

Nice To Haves

  • 1-2 years of acute care, homecare or population health RN case management experience
  • CCM certified
