The Case Manager enhances the delivery of patient care services from admission to discharge through collaboration with the interdisciplinary team. The Case Manager efficiently meets patient needs to assure high quality patient care, reduce length of stay and resource utilization, and ensure reimbursement. Drawing on a broad clinical and systems management knowledge base, the Case Manager facilitates the movement of the patient through the acute phase of an episode of illness and ensures linkage with post discharge care providers. The population served is comprised of adult and pediatric tertiary and ambulatory patients.
