Cooley Dickinson’s VNA & Hospice is also a vital part of our network, providing home-based services throughout Hampshire and Franklin counties. Job Summary Schedule: per diem, scheduled as needed. May include evenings, weekends and holidays. The Case Manager is responsible for providing direction and leadership for patient progression and care coordination. In collaboration with other internal and external disciplines, the Case Manager directs and coordinates the care of patients from admission to transfer or discharge with the goal of cost-effective utilization of hospital resources, minimizing out of pocket expenses for patients, providing appropriate utilization of resources, and patient advocacy. This position reports to the Director, Case Management and Social Work and operates within established organizational and departmental policies and procedures. This position is part of the Massachusetts Nurses Association Bargaining Unit at Cooley Dickinson Hospital.

Requirements

  • Certificate/Diploma Nursing required or Bachelor's Degree Nursing preferred
  • Registered Nurse [RN - Massachusetts] - required
  • Related clinical, continuing care, or case management experience 1-2 years preferred
  • Excellent written and oral communication skills required.
  • Excellent computer skills required including: MS Office, EPIC and 4Next required.
  • Willingness to be flexible, resourceful, and creative in problem solving required; high degree of delegation and prioritization skills required.
  • Excellent interpersonal and relationship building skills to effectively collaborate with multiple stakeholders to obtain staff and physician involvement and accountability in achieving specific patient outcomes required.
  • Demonstrated ability to orchestrate many activities and manage multiple priorities required.
