Case Manager (RN)
About The Position
In collaboration with the patient/family, social workers, physicians, interdisciplinary team, and payer representative, the Care Manager-RN ensures patient progression through the continuum of care in an efficient and cost effective manner. Identifies, initiates and manages optimal care coordination and discharge planning to develop patient flow/throughput to enhance continuity of care, smooth and safe transitions, patient satisfaction, patient safety and length of stay management. Provides support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. Monitors the provision of cost effective care to ensure care is given in the most clinically appropriate setting. Facilitates compliance with regulatory requirements and organizational performance standards, including demonstration of core competencies. Position Specifics: Department: Case Management Core Function: Nursing Schedule: Daytime; M-F; no call; weekend rotation Facility: St. Anthony's Hospital Location: Effingham, IL Compensation that aligns with your experience Education Qualifications Associate’s Degree or Diploma in Nursing is required. Bachelor’s Degree in Nursing is preferred.
Requirements
- Associate’s Degree or Diploma in Nursing is required.
- License as a Registered Nurse (RN) in the state of practice is required.
- Basic Life Support Certification (BLS) is required within 60 days of hire.
- 2 years of clinical experience is required.
Nice To Haves
- Bachelor’s Degree in Nursing is preferred.
- Prior case management experience is preferred.
- Quality/Process Improvement experience is preferred.
- Case Management certification (ACM, CCM, RN-BC) is preferred.
