About The Position

The RN Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive/complex/ safe discharge plans. Collaborates with MSW for guidance with psychosocially complex cases. The RN Case Manager is also responsible for facilitating coordination of care. The RN Case Manager along with the other members of the healthcare team accepts responsibility, authority, and accountability for management of care. This shall be in accordance with the current policies and procedures, mission, vision, and values of UVA Health Northern VA & Culpeper. Serves as a resource for discharge planning and care coordination for medically complex cases. Maintains current knowledge of available community resources/post-acute care options. The incumbent may be asked to perform additional duties as assigned.

Requirements

  • Education: Associate's Degree in Nursing required.
  • Experience: 2 or more years of healthcare experience preferred.
  • Licensure: Current RN licensure in state of employment required.
  • Additional Skills/Requirements Required: Weekend and holiday coverage.

Nice To Haves

  • 4 Year/Bachelor’s Degree, BSN, preferred.
  • Case Management or related experience preferred.
  • Knowledge/Competence of Microsoft Office products and keyboarding.

Responsibilities

  • Patient Care: Interviews and advises patients regarding health and illness prevention.
  • Synthesizes data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.
  • Recommends community resources to meet patient and family needs.
  • Quality: Collaborates with other departments or disciplines to achieve desired quality outcomes.
  • Monitors quality of core processes in department.
  • Applies the principles of safety and infection control to ensure the safety of all patients.
  • Communication: Demonstrates superior interpersonal and communication skills.
  • Works within a team to achieve patient and team goals.
  • Shares and initiates regular and professional communication with co-workers.
  • Participates in regular staff meetings.
  • Identifies opportunities to improve processes based on feedback from staff and customers.
  • Medical Knowledge: Provides primary health care and performs selective medical services under the direction of practice physicians.
  • Obtains complete history and physical data on patients.
  • Reviews patient records.
  • Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures needed.
  • Performs a complete physical exam and records findings.
  • Establishes presumptive diagnosis.
  • Orders appropriate laboratory and diagnostic procedures.
  • Performs independent evaluations and treatment procedures in emergency situations.
  • Provides monitoring and continuity of care between physician visits.
