About The Position

The Case Manager strives for excellence in direct patient care by building a collaborative caring work environment coordinating care and information across the various disciplines focused on patient outcomes and wellness. Assesses plans and facilitates options and services which best meet the patients' healthcare needs in the continuum of care. Formulates goals with the patient and their families. Advocates for the patient through communication and evaluation of available resources to promote quality cost-effective and safe outcomes. Incorporates standards of care for Case Management including identification/selection of patients for case management services, problem identification, planning, monitoring, evaluating, transition of care and outcomes. Implements practice based on current research and evidence-based practice and ensures adherence to hospital and department policies, regulatory standards, and guidelines. Supports an environment of caring focused on the patient's healing and wellness. Ensuring Safety, Compliance and Patient Satisfaction. Delegation of nursing care activities and provisions of supervision; establishes and maintains a plan for the delivery of nursing care; assures that nursing care interventions and activities are directed to resolve problems; and performs technical aspects of care competently. Has access to medication storage areas as required for job tasks. Follows Hospital and Departmental Policies and Procedures.

Requirements

  • Current RN licensure recognized by the Kentucky State Board of Nursing.
  • Bachelor of Science in Nursing.
  • 5 years' experience in acute nursing, case management, utilization management or related field.
  • Completion and maintenance of BLS.
  • ACLS & PALS (or ENPC) within 6 months of hire.
  • NIHSS within 3 months of hire.
  • Successful completion of all competencies as identified on the departmental nurse competency grid.
