The Case Manager (RN or Social Worker) at Absolute Healthcare Resources is responsible for providing case management services to EPD Waiver beneficiaries in their homes, ensuring compliance with EPD Waiver regulations in Washington, DC. This role involves conducting comprehensive assessments, developing and implementing Person-Centered Service Plans, and maintaining communication with beneficiaries and their families to support their health and social needs.

  • Provide ongoing direct observation of beneficiaries and maintain contact with the recipient's Authorized Representative (AR).
  • Conduct comprehensive assessments of the beneficiary's medical, social, and functional status.
  • Develop, present, and implement the Person-Centered Service Plan (PCSP) for the beneficiary.
  • Observe, record, and report changes in the beneficiary's emotional and social factors affecting their illness and need for services.
  • Consult with the attending physician regarding changes in Level of Care (LOC) and Change in Condition requests.
  • Review and respond to all directed tasks in a timely manner and ensure resolution of concerns.
  • Maintain and submit clinical records as required by DHCF, including assessments and communication notes.
  • Evaluate the beneficiary and family's response to EPD Waiver services.
  • Assist in the admission process by evaluating the client's psychosocial status and home environment.
  • Participate in agency meetings and staff development activities.
  • Supervise the Case Manager Assistant/Aide when applicable.
  • Provide community resource planning and crisis intervention.
  • Coordinate with multiple providers and assess appropriateness for program participation.
  • Licensed Social Worker (LGSW or LCSW) or Registered Nurse (RN) in the District of Columbia.
  • Two years of professional experience preferred, with at least one year of experience in home care.
  • Current CPR certification.
  • Strong writing skills and ability to communicate effectively in English.
  • Experience with EPD Waiver regulations and community resources for beneficiaries.
  • Knowledge of social, emotional, and environmental factors related to health problems.
  • Competitive salary ranging from $70,000 to $80,000 per year.
