About The Position

La Rabida Children’s Hospital provides specialized, family-centered health care to children with medically complex conditions, disabilities, and chronic illness. Through expertise, compassion, and advocacy we help children and their families reach their fullest potential, regardless of their ability to pay. Our not-for-profit hospital, licensed for 49 beds, helps transition children from neonatal or pediatric intensive care to home, by providing medical, rehabilitative and developmental care, and by training families to continue treatments and manage the necessary equipment in the home. La Rabida also provides extensive rehabilitation for those recovering from wounds or burns and treatment for exacerbations of chronic conditions. The hospital’s enhanced pediatric patient-centered medical home provides primary care to children with complex medical conditions and their siblings. Children with medical homes elsewhere come to La Rabida for specialty services. La Rabida offers a wide range of specialty services provided to children with sickle cell disease, diabetes, and many others. Children are supported in their emotional and developmental growth, particularly in cases where such growth has been interrupted by accident or disease. Finally, La Rabida provides forensic and treatment services for children exposed to abuse and neglect, comprehensive assessments for wards of the state, early intervention for children between 0 and 3 years of age. Care coordination services for medically complex children are also provided for those who are covered by a health plan and receive care from providers in Cook County Job Summary The Inpatient Case Manager coordinates and manages care for pediatric patients and their families throughout the hospitalization and discharge process. This role works closely with families, medical providers, and community partners to ensure patients receive appropriate care, education, and support while meeting La Rabida standards and policies.

Requirements

  • Bachelor of Science in Nursing Required.
  • 3 years experience in pediatric nursing required.
  • Current IL Professional Nursing License
  • CPR required. (Only certifications by the American Heart Association or American Red cross accepted)
  • Excellent communication and interpersonal skills demonstrated knowledge and expertise in pediatrics and any clinical specialty.
  • Demonstrated knowledge, expertise, and the ability to teach various healthcare topics to families.
  • Organized and motivated self-starter being able to work independently while collaborating effectively as a team member with other healthcare professionals required.
  • Experience in Microsoft Office, Word/Excel required.

Nice To Haves

  • Master of Science in Nursing preferred.
  • 1 year experience in Case Management or a clinical specialty preferred
  • APN license if appropriate
  • Certification in Pediatric Nursing, Case Management, or Clinical Specialty preferred.
  • Ability to speak Spanish fluently highly desirable.

Responsibilities

  • Coordinate and manage day-to-day case management activities for pediatric patients and families
  • Collaborate with patients, families, caregivers, and interdisciplinary teams to develop and update individualized plans of care
  • Assess patient and family needs, including medical, developmental, and educational needs
  • Provide education and resources to families to support care and discharge planning
  • Monitor patient progress and modify care plans as needed, including follow-up between hospitalizations or clinic visits
  • Serve as a liaison between the hospital, families, and external agencies or service providers
  • Assist with care coordination documentation, including treatment plans, prescriptions, and school or service forms
  • Act as a clinical resource to staff and community partners within areas of expertise
  • Track and document patient outcomes and participate in quality improvement initiatives
