About The Position

The Case Manager RN in Home Health is responsible for the overall management of patients, providing direct care while adhering to designated visit productivity standards. This role involves planning, organizing, and directing patient care services for an assigned caseload, ensuring compliance with current standards and regulations to maintain high-quality care. The RN coordinates with an interdisciplinary team to implement patient care plans and manage visit utilization and expenses effectively.

Requirements

• Graduate of an accredited School of Nursing.
• Registered Nurse (RN) license in California.
• Basic Life Support (BLS) certification.
• Valid Class C Driver's License.
• Automobile insurance coverage.
• Minimum of 2 years of recent relevant experience in nursing.

Nice To Haves

• Knowledge of patho-physiology and treatment protocols for common home care diagnoses.
• Understanding of palliative care for terminally ill patients.
• Ability to function independently and as part of a team.
• Demonstrated effective written and verbal communication skills.

Responsibilities

• Manage overall patient care for assigned caseload.
• Provide direct nursing care to patients in accordance with established standards.

• Ensure effective management of visit utilization and control of expenses.
• Assist in agency-wide orientation and competency evaluations for clinical staff.
• Demonstrate clinical assessment skills and recognize special needs of patients, including those in hospice care.
• Communicate effectively with patients and their families in a caring manner.
• Maintain professional relationships with internal and external customers.
• Solve unique problems as they arise and consult with supervisors when necessary.
