As our Case Manager RN, you will meticulously coordinate patient care and services across the entire illness continuum. You will ensure the effective utilization and judicious monitoring of healthcare resources, and collaboratively lead the interdisciplinary team to achieve superior patient outcomes through integrated case management practices.

Requirements

  • Graduate from an accredited school of nursing
  • Registered Nurse: KY, upon hire
  • Knowledgeable in community resources and alternate care facilities

  • 2 years of recent varied acute hospital experience

Responsibilities

  • Meticulously coordinate patient care and services across the entire illness continuum
  • Ensure the effective utilization and judicious monitoring of healthcare resources
  • Collaboratively lead the interdisciplinary team to achieve superior patient outcomes through integrated case management practices
  • Perform a wide variety of high-level patient care activities to include admission assessment, screen for appropriateness, and development of a discharge plan
  • Accomplish timely and appropriate discharges by evaluating the patient's post-discharge needs and the provision of services necessary to meet those needs
  • Comply with safety and emergency procedures
  • Comply with Hospital/Department/Unit policies
  • Document activities consistent with Hospital/Department/Unit Guidelines
  • Perform critical/frequent procedures or services
  • Provide care/services based on identified needs
  • Use required equipment for providing care/services
