About The Position

Requirements

• Current Michigan Nursing License.
• BLS preferred.
• Certification in Case Management preferred.
• Minimum three years of nursing experience in an acute care setting.
• Experience in acute care case management, discharge planning, and utilization management preferred.
• Graduate of an accredited Nursing program.

Responsibilities

• Conduct preliminary risk screenings and assess patients and families' psychosocial risk factors.
• Perform comprehensive assessments of patients’ medical conditions and support systems.
• Provide intervention in cases involving child abuse, domestic violence, and other abuse types.
• Serve as a resource for counseling and intervention related to treatment decisions and end-of-life issues.
• Develop long and short-term plans with patients and families to address identified problems.
• Provide emotional support to help patients cope with their illness or injury.
• Coordinate and facilitate patient care progression throughout the continuum.
• Maintain active communication with interdisciplinary team members for timely patient management.
• Identify and resolve delays and obstacles to discharge.
• Facilitate transfers to other facilities as appropriate.
• Monitor and facilitate length of stay and resource use for at-risk populations.
• Provide clear and timely written documentation and communicate with post-acute care providers.
