About The Position

The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco. The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services. We advocate for health equity and deliver innovative, high-quality services responsive to the neighborhoods and diverse communities we serve. We are currently looking for a qualified candidate for our Case Manger role. This position reports to the Resource Center Program Director, and is a regular, full time, non-exempt role with an hourly range of $27.46 to $29.52 with full benefits.

Requirements

  • Bachelor’s degree in social work, Psychology, Counseling or a related field preferred.
  • Bilingual Spanish/English Required.
  • At least two years of experience as a case manager working with unhoused adults and dually and triple-diagnosed individuals required.
  • Knowledge of motivational interviewing, cognitive behavioral and health psychology.
  • Must be willing to provide support, advocacy and develop relationships following harm reduction principles with a very diverse homeless population.
  • Ability to work with de-escalation techniques, conflict resolution and emergency response.
  • Excellent interpersonal communication and writing skills.
  • Ability to work professionally and ethically in a multi-cultural team setting, including ability to maintain a high level of confidentiality.

Responsibilities

  • Perform individual comprehensive assessment of Resource Center clients’ psychosocial history, mental health status, medical needs, housing, food and other needs for basic support.
  • Determine eligibility and necessity for case management, medical, and psychotherapy and psychiatric treatment, if appropriate.
  • With clients’ assistance, develop care plan delineating steps necessary to accomplish activities, goals and objectives.
  • Implement the plan through the provision of interventions, coordination with integrated services team and/or through facilitated referrals.
  • Utilizing a participant-centered, harm reduction model of services, provides insight oriented, behavior modifying and/or supportive psychotherapy to individuals, families and groups of patients.
  • Provides crisis intervention and assesses clients for suicide, danger to others due to mental illness or grave disability.
  • Utilizes motivational interviewing, cognitive behavioral and health psychology treatment methods.
  • Utilizes depression screening tool (PHQ-9) and other evidence-based screening tools as appropriate.
  • Follows the SAMHSA SBIRT (Screening, brief intervention, referral and treatment) model for patients with substance use problems.
  • Refer clients to appropriate shelter case management beds (Senior, Jazzie’s Place).
  • Assist clients to find, apply, access and stabilize in housing.
  • Assist clients in compiling documentation and filling out forms for immigration and public benefits’ eligibility.
  • In conjunction with Program Manager, Integrated Services assign lockers to clients and provide locker rules and locker contract.
  • Receives and documents monthly locker data in accordance with protocol.
  • Assist clients to connect with MNRC treatment team and to access the San Francisco medical and behavioral health system programs as needed.
  • Attend appointments with clients if needed such as DMV, or SS.
  • Create a hospitable environment and provide services to clients with the utmost dignity and respect.
  • Maintain documentation for case management activities as required by funding sources.
  • Regularly review files and monitor activities of clients with follow-up contact in-person or via phone.
  • Attend relevant meetings, including staff meetings and staff training.
  • Other duties as assigned or required.
