WHAT WE'RE LOOKING FOR If you meet the following criteria, we want you as our Case Manager - Resident Advocate! Works well as part of a team Excellent communication and interpersonal skills Experience in a skilled nursing/rehab facility is a plus, but not required. DAY-TO-DAY This is a part-time position working 20–25 hours per week , scheduled every Tuesday and Friday , plus one flexible additional day . As our Case Manager - Resident Advocate, you'll become a trusted guide for residents and families as they prepare for the next step in their care journey. Each day, you'll coordinate discharge planning, connect individuals with vital community resources, and manage insurance-related needs with accuracy and empathy. You'll collaborate closely with nurses, therapists, and leadership to ensure every resident feels informed, supported, and confident throughout their stay.
