About The Position

The Case Manager/Rehabilitation Specialist position is a temporary, full-time role within the Step Forward North Inland program in Escondido, focusing on providing behavioral health services to CalWORKS participants. The role combines elements of both case management and rehabilitation specialist duties, aimed at assisting clients in overcoming barriers to employment and achieving self-sufficiency. The position requires conducting needs assessments, assisting clients with various applications, providing psychosocial services, and developing relationships with community resources to support clients' vocational goals.

Requirements

  • Bachelor's degree in a mental health-related field, such as Psychology, Social Work, or Counseling required.
  • Minimum of 2 years of experience assisting hard-to-place populations, including severe mental illness, severe emotional disorder and WTW clients.
  • A valid California driver's license in good standing and eligibility to be an approved CRF driver is required.
  • Ability to pass criminal/background clearance.

Nice To Haves

  • Bilingual Spanish preferred, but not required.
  • Intermediate computer skills, including MS Office.

Responsibilities

  • Conduct needs assessments of new/returning clients; perform collateral interviews.
  • Assist clients and families with SSI applications, obtaining housing, childcare, medical/dental appointments, and completing any other forms and applications as needed.
  • Assist clients with transportation, including how to use public transportation, learning routes, or providing rides.
  • Provide individual and group psychosocial services and activities for clients and/or members of client's family.
  • Conduct employment assessments and assist clients in linkage to school, volunteering, or employment opportunities.
  • Work with various businesses and agencies in the community to build relationships for the purpose of job development; help clients build employer contacts.
  • Attend community meetings related to vocational services.
  • Assist clients with developing educational, employment or vocational goals; continually monitor progress towards goals and support client's efforts toward self-sufficiency.
  • Lead clients in employment readiness and employment support groups.
  • Ensure services are in compliance with Work First philosophy.
  • Develop relationships with Social Security Administration, Department of Rehabilitation, Employment Development Department, and other related resources for job development.
  • Educate team members on vocational services to increase the quality of services to clients.
