The Case Manager will function in a collaborative process with patients, nurses, social workers, physicians, caregivers, and the community to facilitate care along a continuum through effective resource coordination. The role involves securing authorization/certification for inpatient services and achieving positive patient outcomes in accordance with quality monitoring, including patient care protocols, evidence-based standards of care, and patient and family guidance. The Case Manager's framework consists of interaction, assessment, planning, implementation, and evaluation, with desired outcomes continually monitored and reassessed to improve patient services. The collaborative effort with a multidisciplinary team ensures that care provided is safe, effective, efficient, client-centered, timely, and equitable. The Case Management process aims for optimal health, access to care, and appropriate resource utilization balanced with the patient's right to self-determination.
