About The Position
Coordinates case management services to at-risk clients, including comprehensive intake and eligibility determination, needs assessment, provision and tracking of referrals and development of positive relationships with community service providers and providing community outreach services. Interviews and counsels at-risk clients by assessing their needs and providing services. Salary will be determined depending on qualifications of candidates who exceed the minimum requirements as outlined within the job description. Additional compensation based on licensure.
