PATH is seeking candidates passionate about helping others make a positive change in their lives to join our SSVF Team as the Case Manager at the RHC Office. As part of the SSVF Team, the Case Manager role’s main function is to assist clients in breaking the cycle of homelessness by utilizing a 'whatever it takes' approach. The position involves conducting intake and individualized needs assessments, developing Individualized Service Plans (ISP), providing referrals and support, completing program-specific assessments, and maintaining documentation standards as required by the Program and PATH policies.
