Our Case Manager / Social Worker plays a vital role in our resident and guest outcomes and overall experience. We are looking for a skilled Case Manager / Social Worker to become a part of our facility's compassionate and hospitable care-giving team.

Requirements

At least one year experience working with elderly population in acute, long-term care, or community-based setting, required
Must be able to read, write, speak, and understand the English language.
Must be able to make independent decisions when circumstances warrant such action.
Compassionate and caring demeanor
Ability to build rapport with residents/families
Ability to have one-on-one conversations, routine visits with residents, and facilitating difficult conversations with families
Demonstrate progressive leadership and initiative
