In partnership with the Department of Health (DHS) and the Office of Diversion and Re-entry (ODR) Team, the ODR Case Manager will work with clients recently released from jail, who are moving from homelessness to permanent supportive housing, to provide intensive case management. Clients represent a diverse group with experiences of complex trauma, and who are facing medical, mental health, and substance use barriers. The Case Manager collaborates with clients to learn independent living skills and other life-skills vital to success in maintaining permanent housing. Clients remain in the program for as long as they require subsidized housing.
