About The Position

Do you want to support young people as they move from crisis and instability toward safety, connection, and long-term housing? Join our Basic Needs team as the Nightly Shelter Case Manager. In this role, you’ll provide flexible, youth-centered case management to young adults ages 18–25 who are experiencing homelessness or housing instability and accessing Spectrum’s Nightly Shelter. The Nightly Shelter Case Manager reports to the Director of Basic Needs and is responsible for providing flexible case management to youth accessing the shelter. They will provide case management and support to youth through assessments, care team coordination, case planning, and access to housing, healthcare and other benefits for youth experiencing homelessness or housing instability ages 18-25. Case management services will emphasize accessing housing, jobs, education, and mental health support. The ability to multitask, collaborate, be flexible, maintain attention to detail, and complete paperwork in a timely manner is essential to this position. The Case Manager, in coordination with the Nightly Shelter Coordinator, works closely with a team of direct service staff in our program and is expected to communicate closely and often with them, and to support their use of appropriate interventions working with the clients. The Nightly Shelter Case Manager will also offer extended care case management to youth living in 4 scattered site apartment units in downtown Burlington, to support housing retention and prevent a return to unsheltered homelessness. The Nightly Shelter Case Manager is expected to participate in the program’s on-call system rotation after hours, with an anticipated cadence of one week every 6-7 weeks, or as needed. This is a nonexempt position with an hourly wage of $25.48 - $29.33 based on experience, plus shift differential.

Requirements

  • A master’s degree in social work (or related field) is preferred, or bachelor’s degree with three years direct case management, or equivalent work experience.
  • Previous experience in providing client-centered, developmentally appropriate services to adolescents, young adults, and their families experiencing complex and varied mental health and substance use concerns.
  • Ability to complete requirements to obtain an Apprentice Addiction Professional Certification.
  • Understanding the complex impact of trauma and experience of providing trauma-informed care.
  • Commitment to the value of diversity with respect to gender, race, ethnicity, age, socio-economic level, sexual orientation, ability, and religion.
  • Demonstrated knowledge and experience with evidence-based practices, such as: Cognitive Behavioral Therapy and Motivational Interviewing.
  • Crisis intervention skills, including suicide risk assessment, safety planning, and resourcing.
  • Knowledge of human development; with special attention to adolescent and young adult biopsychosocial development and family systems.
  • Embraces collaborating with others and enjoys working in an open, supportive, and committed team that is dedicated to supporting youth and their well-being.
  • The ability to utilize supervision by asking for assistance, support and training, and accepting and utilizing feedback.
  • Ability to manage time and set priorities to accomplish tasks and meet deadlines, including the ability to adapt to changing priorities.
  • Ability to work independently and as an effective team member.
  • Valid driver’s license and reliable transportation required.

Responsibilities

  • Assist youth in identifying their goals.
  • Assist youth in developing a housing plan that uses their strengths to help them meet their goals.
  • Assist youth in accessing the Coordinated Entry system.
  • Assist youth in attaining sustainable employment/income.
  • Assist youth in obtaining necessary documentation.
  • Assist youth in identifying their goals to complete secondary education and prepare, enroll, and complete post-secondary education.
  • Assist youth in accessing and maintaining affordable housing including submitting all necessary applications and working with community partners.
  • Assist youth in accessing health care and other benefits.
  • Assist youth in identifying and developing lifelong connections and support from caring adults.
  • Assist youth in housing retention, making appropriate service connections and, when applicable, offering aftercare case management, including for youth living in the Champlain Housing Trust managed Grant Street apartments (4).
  • Represent the program and the agency at inter-agency community meetings intended to improve service delivery and information distribution.
  • Work collaboratively with the Spectrum Central Intake team to make appropriate referrals and connections for youth entering program.
  • Administer SBIRT intake screenings when applicable.
  • Schedule, organize, and facilitate team meetings.
  • Attend all service meetings as designated.
  • Development of detailed service plans.
  • Monitoring & communicating progress related to the service plan/ programming in a timely manner to the team of providers.
  • Co-facilitate designated programming groups as needed.
  • Support training of new shelter staff as needed.
  • Provide support to shelter staff as needed.
  • Maintain client records in both the agency Electronic Health Records system (Foothold) as well as the state-managed HMIS in a timely and accurate manner.
  • Participate in On-Call rotation for Basic Needs Programs.
  • Attend bi-weekly individual supervision with Director of Basic Needs
  • Attend weekly group supervisions of the Basic Needs Leadership Team
  • Attend weekly meetings with Nightly Shelter Coordinator
  • Attend weekly Nightly Shelter team meetings and Basic Needs cross-over meetings
  • Attend semi-monthly community outreach meetings
  • Maintain positive professional relationships with resources, agencies, and providers to coordinate services for youth
  • Engage in required trainings, as well as continuing education to support professional development and ensure high quality of care
  • Collaborate with Nightly Shelter Coordinator and Director of Basic Needs to regularly assess and update program policies
  • Perform other duties as assigned by the Director of Basic Needs
