About The Position

The Case Manager promotes and supports individuals who are at risk of developing or who have mental health and/or substance use issues. Services include assessing, monitoring, and linking to services, supportive level counseling, education, and advocacy. Case management services may also be provided to a small number of priority population women with substance use disorders as needed.

Requirements

  • Bachelor’s degree from an accredited college or university in an applicable human services field.
  • A minimum of one (1) year of relevant mental health, developmental disability, and/or substance use work experience.
  • Knowledge of services, systems, and programs available in the community.
  • Understanding of serious mental illness, developmental disabilities, and substance abuse.
  • Ability to identify and document an individual's needs for resources and services.
  • Experience in treatment modalities and intervention techniques.
  • Familiarity with applicable federal and state laws, regulations, and local ordinances.

Responsibilities

  • Assess, monitor, and link individuals to services.
  • Provide supportive level counseling, education, and advocacy.
  • Conduct functional assessments and use them in service planning.
  • Develop individual service plans based on assessments and evaluations.
  • Coordinate the provision of services by public and private providers.
  • Engage and sustain ongoing relationships with individuals receiving services.
  • Work as part of a team while maintaining effective inter- and intra-agency relationships.
  • Drive and transport clients as needed, requiring a valid Virginia driver’s license.
  • Participate in behavioral intervention training, including verbal de-escalation skills and physical techniques.
  • Work modified schedules, including weekends and holidays, as required.
