About The Position

As a Case Manager, you will be a key member of a multidisciplinary team providing recovery-oriented services to adults with severe and persistent mental illness. Many of our clients also experience co-occurring physical, developmental, or substance use challenges. Your role will focus on enhancing Independent Living Skills and supporting clients in achieving greater autonomy and stability in their daily lives.

Requirements

  • Bachelor’s degree in social work, counseling, psychology, or a related field required; Master’s degree preferred.
  • Minimum one year of experience in the social service field, including internships.
  • At least one year of case management experience working with diverse populations with complex needs.
  • Excellent written and verbal communication skills.
  • Strong decision-making, problem-solving, organizational, and computer skills.
  • Ability to work independently and manage responsibilities autonomously.
  • Valid driver’s license and approval through agency’s insurance carrier to operate agency vehicles.
  • Must pass a Criminal Background Check and Nurse’s Aide Registry (if applicable).

Nice To Haves

  • Master’s degree preferred.
  • Experience with mental health populations and knowledge of Medicaid rules preferred.

Responsibilities

  • Deliver individualized support in areas such as medication management, healthy living, psychosocial needs, and self- management.
  • Monitor client progress and update service/treatment plans as needed.
  • Collaborate with clients, team members, external providers, and support networks to ensure continuity of care.
  • Facilitate therapeutic and recreational group activities both onsite and in the community.
  • Provide referrals and follow-up for housing, entitlements, education, employment, and guardianship services.
  • Conduct assessments (including IM-CANs) and assist with service linkage and plan updates.
  • Maintain accurate documentation in Envision electronic care system and meet productivity targets.
  • Assist with program development, including workflow design and procedure manuals.
  • Participate in supervision, training, and committee meetings; stay current with best practices in case management.
