About The Position

As a Case Manager, you will be a key member of a multidisciplinary team providing recovery-oriented services to adults with severe and persistent mental illness. Many of our clients also experience co-occurring physical, developmental, or substance use challenges. Your role will focus on enhancing Independent Living Skills and supporting clients in achieving greater autonomy and stability in their daily lives.

Requirements

Bachelor’s degree in social work, counseling, psychology, or a related field required; Master’s degree preferred.
Minimum one year of experience in the social service field, including internships.
At least one year of case management experience working with diverse populations with complex needs.
Excellent written and verbal communication skills.
Strong decision-making, problem-solving, organizational, and computer skills.
Ability to work independently and manage responsibilities autonomously.
Valid driver’s license and approval through agency’s insurance carrier to operate agency vehicles.
Must pass a Criminal Background Check and Nurse’s Aide Registry (if applicable).

Nice To Haves

Master’s degree preferred.
Experience with mental health populations and knowledge of Medicaid rules preferred.

Responsibilities

Deliver individualized support in areas such as medication management, healthy living, psychosocial needs, and self- management.
Monitor client progress and update service/treatment plans as needed.
Collaborate with clients, team members, external providers, and support networks to ensure continuity of care.
Facilitate therapeutic and recreational group activities both onsite and in the community.
Provide referrals and follow-up for housing, entitlements, education, employment, and guardianship services.
Conduct assessments (including IM-CANs) and assist with service linkage and plan updates.
Maintain accurate documentation in Envision electronic care system and meet productivity targets.
Assist with program development, including workflow design and procedure manuals.
Participate in supervision, training, and committee meetings; stay current with best practices in case management.
