About The Position

Our home care agency is seeking a Case Manager with Medicaid Managed Care (MCO) experience to support referral growth and relationship development with Sunflower Health Plan, UnitedHealthcare Community Plan, and Healthy Blue. This role is ideal for candidates with prior experience at an MCO as a Case Manager, Care Coordinator, Service Coordinator, or Waiver Specialist who understand HCBS waiver programs, authorizations, and community-based services.

Requirements

• Previous experience with a Medicaid MCO (Sunflower, UHC, Healthy Blue preferred)
• Strong understanding of HCBS programs, waivers, and Medicaid service authorization processes
• Strong understanding of FE and PD, waivers
• Experience in case management, service coordination, or provider relations
• Valid driver’s license and reliable transportation
• Excellent communication and relationship-building skills

Responsibilities

• Build and maintain strong relationships with MCO case managers, care coordinators, and provider relations representatives
• Identify and develop referral opportunities for home care and home health services
• Present agency services to MCO teams, hospitals, and community partners
• Assist MCO staff in coordinating services for new and existing members
• Track referral activity and provide weekly updates to leadership
• Attend community events, waiver fairs, and MCO outreach meetings
• Coordinate with internal scheduling and intake teams to ensure timely service starts
• Support members transitioning from hospitals, nursing homes, and HCBS programs into home-based services
