That’s where you come in. As a Case Manager with the Maternal Home Visiting Program, you’ll step directly into homes and lives at their most vulnerable and most hopeful moments. You’ll provide trusted guidance, coaching, and advocacy—whether it’s teaching safe sleep practices, screening for postpartum depression, or helping a family connect to healthcare, WIC, or childcare resources. Every visit becomes more than a check-in. It’s an opportunity to strengthen parent-child bonds, improve health outcomes, and empower families to chart their own futures.
