  • Completes clinical assessments of members to determine eligibility for case management.
  • Develops and implements case management plans in collaboration with members and healthcare professionals.
  • Conducts telephonic, face-to-face, or home visits as required.
  • Monitors care plans to evaluate effectiveness and suggest changes as needed.
  • Maintains an ongoing member case load for regular outreach and management.
  • Promotes integration of services for members, including behavioral health care and long-term services.
  • Facilitates interdisciplinary care team meetings and informal collaborations.
  • Uses motivational interviewing to educate and support members during contacts.
  • Assesses barriers to care and provides coordination and assistance to members.
  • Completion of an accredited Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) Program OR Bachelor's or Master's Degree in a relevant field.
  • 1-3 years of experience in case management, disease management, managed care, or medical/behavioral health settings.
  • Active, unrestricted license in good standing (if required).
  • Valid driver's license with a good driving record.
  • 3-5 years of experience in case management, disease management, managed care, or medical/behavioral health settings.
  • Licensure as a Licensed Clinical Social Worker (LCSW), Advanced Practice Social Worker (APSW), Certified Case Manager (CCM), or similar.
