About The Position

The University of Miami/UHealth Department of Pediatrics has an exciting opportunity for a full-time Case Manager, Licensed to work in Miami, FL. The Case Manager, Licensed coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. The incumbent also provides ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient’s needs. An employee in this position acts as a liaison between patients and families, and healthcare personnel to ensure necessary care is provided promptly and effectively. 50% This position is responsible for providing administrative program support and coordination of service implementation and program operations for the Healthy Start Neonatal Abstinence Syndrome or Substance Exposed Newborn Project (HS NAS/SEN), an initiative focused on reducing the occurrence of in-utero substance exposure and enhance the system of care to families with or at risk of having substance exposed newborns. The position will coordinate and manage clinical program implementation including maternal child health services and community outreach to HS NAS/SEN clients (pregnant women, women of child bearing age, and infants up to age 3 and their families). Position is salaried, and requires working evenings and weekends as needed to ensure that job duties are met. Responsibilities also include administrative oversight of all required staff, data collection, and working closely with supervisors in compliance with all HS reporting requirements.

Requirements

  • Bachelor’s degree in relevant field.
  • Minimum 3 years of relevant experience
  • Required Licensed Mental Health Counselor (LMHC) or Licensed Clinical Social Worker (LCSW)
  • Ability to communicate effectively in both oral and written form.
  • Ability to handle difficult and stressful situations with professional composure.
  • Ability to maintain effective interpersonal relationships.
  • Ability to understand and follow instructions.
  • Ability to process and handle confidential information with discretion.
  • Any relevant education, certifications and/or work experience may be considered.

Nice To Haves

  • Master’s degree in relevant field, highly preferred

Responsibilities

  • Plans, implements, and evaluates case management activities across the continuum of care.
  • Provides patient education to assist with self-management.
  • Evaluates and communicates case objectives and goals to case management teams.
  • Consults physicians to determine the medical necessity and appropriateness of treatment.
  • Evaluates all referral sources and their qualifications to assure quality outcomes.
  • Determines the cost effectiveness and medical necessity of alternate treatment.
  • Negotiates with providers when necessary to reduce patient cost.
  • Adheres to University and unit-level policies and procedures and safeguards University assets.
  • Assists with team management of Housing/Employment Specialist and Peer Specialist activities.
  • Conducts assessments with program participants
  • Coordinates referrals as needed to Housing/Employment Specialist and Peer Specialist.
  • Refer program participants to all necessary services and follow-up accordingly
  • Review program records and ensure compliance with all Healthy Start and NAS/SEN Standards and Guidance and Policy and procedures
  • Supervise of all program, community and health education activities to ensure successful implementation of programmatic functions.
  • Assist the Program Director with management of all quality assurance activities related to service delivery, evaluation data collection, and agency reporting.
  • Evaluates staff productivity, revises procedures as needed, and develops new tracking methods to maximize staff performance and productivity and other workflow issues.
  • Assists the Program Director and coordinates the development, implementation, and evaluation of Healthy Start policies and procedures designed to improve program operations.
  • Ensures HS NAS/SEN employees complete all required maternal child health trainings and mandatory UM trainings related to University policy and procedures.
  • Serve as program liaison to collaborating service providers and community agencies; conduct Healthy Start community outreach activities as needed;
  • Attend all programmatically required trainings and meetings and provide in-service educational presentations/trainings to program staff.
  • Other organizational/support duties as assigned by the Program Director
