The Case Manager Level IV position is responsible for identifying and assessing child and family needs, evaluating, coordinating, and ensuring necessary services are provided. The role includes providing in-home services to clients and assisting and counseling individuals and families by delineating alternatives, helping to articulate goals, and providing needed information. The position requires leading staff meetings, mentoring lower-level case managers, and acting as a unit supervisor in their absence. The Case Manager will also be responsible for critiquing and providing feedback for judicial reviews and case plans, attending staffings for reunification, and leading special projects as requested.

  • Provide case management services including information, plan referrals, and linkages.
  • Develop and maintain case records and service documentation according to contract and Community Partnership for Children standards.
  • Document case activities in FSFN within 24 hours of the event.
  • Complete all required court reports and submit within established deadlines.
  • Testify in court as necessary.
  • Monitor quality of care by conducting face-to-face home/residential visits of the child every 30 days.
  • Coordinate services with other professionals and paraprofessionals.
  • Conduct community outreach as needed.
  • Provide and/or arrange for transportation of clients to appointments and community agencies.
  • Conduct monthly face-to-face visits with parents of children in Out of Home Care to discuss case plan progress.
  • Provide and/or refer families to services and understand information from service providers.
  • Conduct child safety assessments.
  • Serve as on-call case manager after hours and respond to immediate risk and needs of child.
  • Arrange for emergency placement for children at risk.
  • Explain rights and responsibilities to children and family members.
  • Provide/coordinate support to foster homes and relative/non-relative placements.
  • Provide post-placement planning and services.
  • Participate in quality improvement.
  • Bachelors degree from an accredited college or university in human services related field.
  • Attainment of Child Welfare Credentials.
  • Possess a current Background Clearance Screening Letter as required by DCF.
  • Successfully complete the background screening requirements for Community Partnership for Children.
  • Valid Florida driver's license and documentation of current automobile insurance if local travel is required.
  • Two years case management experience, preferred.
  • No written counseling statements within the last 2 years, preferred.
  • Willingness to provide mentoring to level 2 and 3 CM's with a positive team approach attitude.
  • Must attend SFE (Supervising for Excellence) Training within one year of position start date, preferred.
