About The Position

The position is within the Patient Admitting department at Ascension Alexian Brothers Behavioral Health, located in Hoffman Estates, IL. This role is scheduled as PRN/As Needed for Weekend Night Shifts. The salary for this position ranges from $26.25 to $38.39 per hour. The responsibilities include coordinating a comprehensive plan of care for patients, overseeing their primary needs, providing home care/consultation, monitoring patient care, advocating for resources, and maintaining communication with supervisors and health providers.

Requirements

• Licensure / Certification / Registration: Counselor credentialed from the Illinois Department of Financial and Professional Regulation preferred.
• Social Worker credentialed from the Illinois Department of Financial and Professional Regulation preferred.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
• Education: High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.

Nice To Haves

• A Master's Degree in Social Service is required.
• Behavioral Health/Intake experience highly preferred.
• 12-hour Weekend Night Shift 7:00 pm - 7:30 am (Friday and Saturday).

Responsibilities

• Coordinate comprehensive plan of care for patients.
• Oversee primary needs of patient, including assessment of client and family/caregivers, and develops comprehensive plan of care.
• Provide primary home care/consultation to complex client and family needs within area of expertise and in collaboration with physician.
• Monitor patient care through assessments, evaluations and/or patient records.
• Advocate for resources and removal of barriers.
• Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
