Performs highly complex case management work. Work involves reviewing and tracking case management activities; and assisting in developing program goals, objectives, and procedures. Works under limited supervision with considerable latitude for the use of initiative and independent judgment.

  • Provides ongoing case management and reviews and tracks case management activities for substance use inmates.
  • Interviews inmates to gather information to assess service needs and review progress and compliance within the program.
  • Participates in team meetings for individualized program planning, behavioral interventions, and reentry planning.
  • Assists with program monitoring and evaluation; assesses inmates to identify problem areas, service gaps, and areas that are barriers to services.
  • Participates in the quality assurance process.
  • Provides liaison with service providers; assists with Intra-agency and Interagency services coordination and referrals.
  • Documents and maintains program activity and case management records; prepares and reviews reports.
  • Prepares and presents educational materials in individual and group settings.
  • Coordinates activities within the treatment program; carries a caseload for cognitive-based services.
  • Works with the treatment team of licensed chemical dependency counselors, qualified credentialed counselors, security staff, supervision officers, and others to monitor inmate progress.
  • Makes recommendations in treatment planning.
  • Performs a variety of marginal duties not listed, to be determined and assigned as needed.
  • Graduation from an accredited senior high school or equivalent or GED.
  • Three years full-time, wage-earning case processing, case management, social services, criminal justice, human services, counseling, substance abuse treatment, or rehabilitative programming experience.
  • Fifteen semester hours from a college or university accredited by an organization recognized by the Council for Higher Education accreditation (CHEA) or by the United States Department of Education (USDE) may be substituted for each six months of experience.
  • Rehabilitation programming experience preferred.
  • Transitional and reentry planning preferred.
  • Knowledge of case management principles, objectives, standards, and methods.
  • Knowledge of applicable state and federal laws, rules, regulations, and statutes.
  • Knowledge of agency and departmental organizational structure, policies, procedures, rules, and regulations preferred.
  • Knowledge of agency substance use treatment program policies and procedures preferred.
  • Skill to communicate ideas and instructions clearly and concisely.
  • Skill to coordinate with other staff, departments, officials, agencies, organizations, and the public.
  • Skill to interpret and apply rules, regulations, policies, and procedures.
  • Skill in problem-solving techniques.
  • Skill to prepare and maintain accurate records, files, and reports.
  • Skill to review technical data and prepare technical reports.
  • Skill to plan work in order to meet established guidelines.
  • Skill to assess inmate needs, coordinate inmate services, and implement program services.
  • Skill to gain knowledge of agency and divisional core competencies.
  • Skill to model the way for positive change.
  • Rehabilitation programming experience preferred.
